Continuum Health Partners
Continuum Health Partners, a nonprofit hospital system in New York City, recently purchased Craneware software to manage its mainframe charge description master (CDM). A primary goal of Continuum is to utilize the Craneware software for continual monitoring and maintenance of the CDM to improve the billing process. The Capstone team worked with the Department of Charge Capture to analyze existing trends in charge capture, and make recommendations for optimal use of the software. By interviewing hospitals that have already successfully launched Craneware and reviewing Continuum's internal data, the team was able to develop a Craneware User's Guide and a comprehensive report as the final project deliverables.
Elmhurst Hospital Center of the Queens Health Network
Elmhurst Hospitals obstetrics service has experienced a significant decline in the number of obstetrics deliveries in the past several years. The Hospital engaged a Capstone team to analyze OB models of care nation-wide and within the New York Metropolitan Area and to provide suggestions for changes in Elmhursts OB services that would improve patient satisfaction as well as increase new patient referrals. To address the problem, the following objectives were reached: (1) find nationwide obstetrics services/models of care; (2) analyze Elmhurst Hospital obstetrics delivery services; (3) choose relevant models of care; (4) propose changes to the current model of care; and (5) develop a marketing plan to promote obstetrics services available to the community.
Institute for Urban Family Health
The Capstone team worked with the Institute for Urban Family Health (the Institute) to analyze the impact that best practice alerts (BPAs), a function of an Electronic Medical Record (EMR), had on healthcare costs. In order to do this, the Capstone team designed a retrospective study that consisted of a review of pre and post-BPA implementation data between the years 2002-2005. The data was acquired from Continuum Health Care and contained utilization and financial claim data associated with the Institutes diabetic patient population. Once the data analysis portion of the study is completed, it will be determined whether or not there is a cost savings associated with BPA usage. These results can then be used to aid the Institute in developing future quality improvement initiatives and to guide the allocation of scarce resources. Furthermore, the results have the potential to persuade other community health care centers to adopt EMRs.This Capstone was made possible with the support from an anonymous donor through FJC: A Donor-Advised Fund.
Jewish Home and Hospital Lifecare System
The Jewish Home and Hospital Lifecare System (JHHLS) is a complex nonprofit long term care system that serves almost 10,000 elders annually. JHHLS is committed to serving the needs of its residents and clients. JHHLS regularly surveys its staff and residents to evaluate their level of satisfaction and to continually provide the highest quality of care for all elders, staff, caregivers and their families. The Capstone team conducted an extensive literature review, examined all existing data, made connections between staff and resident satisfaction and developed a tool in order to help JHHLS understand what relationships exist between the staff and elder satisfaction. The tool will help study relationships on an on-going basis. The Capstone team has also made recommendations towards improving staff and elder satisfaction surveys.
New York Zero-to-Three Network
The New York Zero-to-Three Network (NYZTT) promotes the optimal development of young children, their families, their communities and the systems that serve them in the New York City region. The Capstone team was asked by NYZTT to create an advocacy tool to bring political focus to the problems facing the zero-to-three population in New York City and New York State. The team engaged in an extensive literature review and data collection on key topics such as health, early care/education, economics, child welfare, mental health and safety. In addition, the team contacted numerous agencies in the state and city government and spoke with NYZTT Board members who are specialists in these key areas. Multiple forms of dissemination were researched to find the most effective method to present this information. From this research, the team identified the most compelling indicators and statistics and developed it into a comprehensive report card. This report card will be used to educate and lobby New York City and New York State legislators as well as the general public into making young children a priority by investing in key, proven programs and services focused on our young children.
