NYU Reynolds Program in Social Entrepreneurship Undergraduate Scholarship Application Intent Form 2011

Directions

The following form serves two purposes:

• Notification of your intent to complete an application for an NYU Reynolds Undergraduate Scholarship and
• Notification for your recommenders of this intent and to provide them with instructions on how to complete their portion of your application.

Please note: This form is required in order for you to proceed with your application and must be completed before you may submit your application. Your recommenders will not be able to complete their portion of your application until you have completed this form.

Following the completion of this form, you will receive an email confirmation. The confirmation will include a unique applicant ID and a link to the application form. You will need the applicant ID and link in order to complete Part II of the application.

Recommendation Information

Please provide the names and email addresses of two recommenders who will be submitting a letter of recommendation on your behalf. Two recommendations are required. We strongly suggest one recommendation comes from a faculty or staff member from within the NYU community and one from an individual who can speak directly to your current and/or potential role as a changemaker, who is outside of the NYU community.

After you submit this form, each recommender will automatically receive an email with instructions on how to complete and submit their recommendations. Your recommenders must complete their recommendations online and by the Undergraduate Scholarship Application deadline of January 21, 2011. This document outlines the information the recommender will be asked to complete on their online form and may be printed and provided to them beforehand.
