Elmhurst Hospital Center, Queens Health Network

The Resident was involved in many ongoing and new projects within the Business Development and Strategic Planning Department. The Resident participated in existing and new disease management teams and assisted them by analyzing data to segment the patient population, develop outcome targets, and stratify interventions according to each group’s acuity. The Resident also compiled a pac-age of tested and implemented changes across the domains of the chronic illness model to document practice changes which have been adopted by each team ,have been demonstrated to be effective, and are poised for widespread use. In addition, the Resident was involved in writing a grant proposal to study the effects of Care Management on ED Utilization.
