The Capstone team assessed the feasibility of a shared information network among geriatric service providers in Northern Manhattan. Information sharing among providers is currently informal and incomplete. This creates difficulties in coordinating care, making referrals, and rendering the highest quality of care. To determine the feasibility of a shared network, the Capstone team interviewed a select group of providers and identified their needs and willingness to participate; researched the conditions under which information sharing is legal according to the Health Insurance Portability and Accountability Act of 1996 (HIPAA); explored the successes and failures of Community Health Information Networks (CHINS); and defined viable technology options. As a result of this research, the Capstone team determined that agencies are willing to participate in a network and it is feasible so long as certain legal precautions and security measures are taken.
