Institute for Urban Family Health
The Institute for Urban Family Health is a not-for-profit health care organization whose mission is to provide quality health care for urban populations. Led by the Institute for Urban Family Health and several community-based organizations and faith groups, Bronx Health REACH is a CDC-funded project to end racial and ethnic disparities in health care and health outcomes for diabetes and heart disease in the southwest Bronx. The resident provided programmatic and operational support to the following key initiatives that comprise Bronx Health REACH: Faith-Based Outreach Initiative; Community Health Advocates; Nutrition and Fitness Initiative; Public Health Education and Outreach Initiative; Legal and Regulatory Workgroup; Health Care Providers Workgroup; and Community Household Survey.
