The Institute for Urban Family Health is a not-for-profit health care organization whose mission is to improve the quality and availability of family practice services in response to the needs of medically underserved populations. The resident was primarily involved in Bronx Health REACH, a project developed by the Institute in 1999. Bronx Health REACH is a coalition of organizations whose goal is to eliminate racial and ethnic disparities in health outcomes in the Southwest Bronx, focusing on diabetes and heart disease. In this capacity, the resident provided programmatic and operational support to the following initiatives: the development of a statewide advocacy agenda, the planning of a health equality advocacy day in Albany, a public health education and outreach campaign, and a quality improvement project for diabetes care at the Institute’s health centers.
