The Capstone team was asked by the Department of Juvenile Justice (DJJ) to develop an integrated discharge planning model, from intake to detention, for detained juveniles. To help develop this model, the Capstone team researched trends that affect discharge planning and the best practices of integrated discharge planning nationwide. The capstone team also interviewed DJJ divisions and external agencies that assess juveniles’ needs and plan juveniles’ services. From the information collected in these interviews, the Capstone team analyzed DJJ’s information collection and transfer. The Capstone team provided recommendations on integrated discharge planning models appropriate for DJJ, developed a flow chart of DJJ’s information flow system, and provided recommendations to improve DJJ’s information flow system.
